We are currently in need of a Licensed Practical Nurse. As a LPN, you will be responsible for delivery of Health Maintenance Activities and Care to the Residents The Veranda . The LPN performs all other duties necessary to meet the individual needs as outlined in the Resident Care Plan and on the MAR. All job functions are performed with customer service and hospitality as the overall goal to both our residents and coworkers alike.

Essential Functions:

1. Works using the guidelines established by the Department of Community Health and company’s policies and procedures.
2. Assesses Residents experiencing a Change in Condition and ensures that the appropriate care is put in place
3. Makes rounds of the buildings reading the communication book, following up with residents who have experienced a change in status.
4. Communicates with residents’ families/Responsible party and the resident’s physician when a change in status occurs.
5. Provides direction written and verbal to Resident Care Assistance as it relates to the care of the Residents.
6. Participates in monthly assessment meetings.
7. Responds to medical emergencies involving independent and assisted living residents.
8. Provides support to the RA’s in providing direct care to the residents.
9. Gives and receives report from and to on call staff member and RSP at shift’s end.
10. Conducts self in a professional manner in compliance with facility policies.
11. Performs Health Maintenance Activities including assistance with medication, B-12 injections, minor wound care and other HMAs within the scope of the rules and regulations.

Qualifications:

1. Graduation from a basic education program for LPNs.
2. A Current LPN license with no disciplinary action against the license.
3. Eligible as determined by FDLE background screen
4. Ability to read, write, and speak English fluently
